To achieve these goals, the individual needs are recorded as comprehensively as possible.

When a new client is accepted, an initial collection of information is created based on an anamnesis, which should provide information about the care situation and the resulting range of activities.

Afterwards, our specialists create appropriate care plans that are based on the activities and existential experiences of life.

The planning of the care is basically based on the existing resources and skills of the clients, the care measures on the needs and necessities. The principle goal is the best possible improvement and maintenance of the quality of life of the individual.

The established goals are regularly checked as part of the revision of our care plans and redefined if necessary.

If necessary, our team of nursing staff can provide psychosocial support. In addition, advice is given to clarify the cost of care, to design personal surroundings and, if necessary, other cooperation partners.

Relatives of clients are, as far as possible, integrated into the nursing process; if necessary, advice and training take place here as well.

The cooperation and the regular exchange of information with the respective treating physicians are involved in order to optimize the care.

Nursing aids are organized as quickly as possible through cooperation partners if required.
